Manejo quirúrgico de la fisura alveolar en pacientes con fisura labiopalatina. Revisión narrativa

Un importante número de los pacientes con fisura labiopalatina presentan una discontinuidad del hueso maxilar en la región alveolar, la cual puede manifestarse de manera unilateral o bilateral y se denomina fisura alveolar. Este defecto reviste gran importancia clínica, dado que compromete la erupción dentaria al carecer de un soporte óseo adecuado. Su persistencia se asocia con diversas complicaciones, entre ellas la formación de fístulas oronasales, la aparición de asimetrías faciales, alteraciones en la función fonatoria, así como hipoplasia o ectopia del incisivo lateral. La corrección quirúrgica puede llevarse a cabo en distintos momentos evolutivos y mediante diferentes técnicas; sin embargo, la osteoplastia secundaria con injerto óseo, realizada antes de la erupción del diente adyacente a la hendidura, constituye el procedimiento más empleado. El injerto óseo autólogo es considerado el material de elección por su capacidad de favorecer los procesos de osteogénesis, osteoinducción y osteoconducción. Los sitios donantes más utilizados corresponden a la cresta ilíaca y la región parasinfisaria. El abordaje integral de la fisura alveolar requiere una estrategia multidisciplinaria y la aplicación estricta de los principios terapéuticos en cada etapa del proceso clínico, con el propósito de garantizar resultados funcionales y estéticos óptimos.
